Identity Verification Completed With Notary
This form is for students unable verify identity by presenting unexpired valid government issued photo identification in-person or through live video call.
This form and the Identity Verification Documents referenced within must:
Documents and/or forms notarized using online/virtual notary services or the foreign equivalent of a notary public cannot be accepted.
Forms with notary affirmation must be submitted to your schools Financial Aid Office using a traditional mail service and must include a copy of the Identity Verification Documents presented at the time of notarization; forms and documents submitted in a non-electronic format cannot be accepted.
Forms must be completed in full and received without omission or error; forms missing information, wet signature(s), required copies of the Identity Verification Documents referenced within, and/or without having been notarized cannot be accepted.
