This form allows students to notify the university that they have or will cease attendance in pursuit of withdrawing or taking a leave of absence from school.  If you are considering taking time away from school on a temporary or permanent basis, please review the information at your disposal for important information that may assist you in pursuing the option that best meets your needs.
